Terms of Use

These Terms of Use (“Terms”) govern your access to and use of the LiveWell Medical website, including livewellmedical.org and any webpages, forms, features, content, or services made available through the website (collectively, the “Site”).

The Site is owned and operated by LiveWell Medical (“LiveWell Medical,” “LiveWell,” “we,” “us,” or “our”).

By accessing or using the Site, you acknowledge that you have read, understood, and agreed to these Terms. If you do not agree to these Terms, please do not use the Site.

1. Website Information Only

The information presented on this Site is provided for general informational and educational purposes only.

Nothing on the Site is intended to:

  • Diagnose, treat, cure, or prevent any disease or medical condition;

  • Replace advice, diagnosis, or treatment from a qualified healthcare professional;

  • Serve as individualized medical advice;

  • Create a physician-patient relationship; or

2. Medical Emergencies

The Site, its contact forms, email addresses, and other online communication features are not intended for medical emergencies or urgent medical concerns.

If you believe you are experiencing a medical emergency, call 911 or go to the nearest emergency department immediately.

Do not use the Site, email, or a general contact form to communicate urgent symptoms, emergency concerns, or time-sensitive medical information.

3. No Physician-Patient Relationship

The Site may provide information about services such as:

  • Concierge medical care;

  • Preventive and wellness medicine;

  • Anti-aging and longevity medicine;

  • Men’s and women’s health;

  • Bio-identical hormone replacement therapy;

  • Nutritional and metabolic optimization;

Availability, eligibility, pricing, risks, benefits, and expected outcomes vary by patient and service. All medical services are subject to an appropriate consultation, medical history, evaluation, clinical judgment, applicable laws, and professional standards.

Not every individual will qualify for every service. LiveWell Medical does not guarantee any specific medical, wellness, aesthetic, performance, weight-management, anti-aging, or longevity result.

5. Concierge Memberships

Information displayed on the Site regarding concierge memberships, enhanced access, appointment availability, physician communication, pricing, or membership benefits is provided for general informational purposes.

Specific membership services are governed by the applicable membership agreement, patient agreement, financial policy, informed consent, and other documents provided by LiveWell Medical.

If these Terms conflict with a separately signed patient or membership agreement, the signed agreement will control regarding the subject covered by that agreement.

Membership in a concierge medical practice is not health insurance and should not be represented or relied upon as a substitute for health insurance.

6. Appointment Requests and Communications

Submitting an appointment or consultation request does not guarantee that:

  • An appointment has been scheduled;

  • A requested date or time is available;

  • LiveWell Medical has accepted you as a patient;

  • A particular physician or service will be available; or

  • LiveWell Medical will respond within a particular timeframe.

 

An appointment is confirmed only when you receive confirmation directly from LiveWell Medical.

You are responsible for providing accurate and current contact information. LiveWell Medical is not responsible for communications that you do not receive because your information was incomplete, inaccurate, or outdated.

7. General Contact Forms and Email

General website forms and standard email may not be appropriate for transmitting confidential, highly sensitive, or urgent medical information.

Please do not submit detailed medical histories, Social Security numbers, financial account information, insurance identification numbers, photographs of medical conditions, or other highly sensitive information through a general contact form unless LiveWell Medical specifically directs you to an approved secure method.

Communications sent through the Site are governed by our Privacy Policy and, where applicable, LiveWell Medical’s Notice of Privacy Practices.

8. Accuracy of Website Content

LiveWell Medical makes reasonable efforts to provide helpful and accurate information. However, medical knowledge, clinical standards, laws, treatments, product availability, service offerings, and pricing may change.

We do not warrant that Site content is complete, current, accurate, error-free, or appropriate for every individual.

LiveWell Medical may modify, correct, remove, or update Site content at any time without notice.

  • Reliance on general Site content;

  • Errors, omissions, interruptions, or security incidents;

  • Third-party websites or services;

  • Unauthorized access to communications; or

  • Decisions made based on Site information.

 

Nothing in this section limits liability that cannot lawfully be limited or excluded.

These limitations relate to use of the Site and do not modify any rights or responsibilities arising from professional medical services provided through an established physician-patient relationship.

17. Indemnification

To the extent permitted by law, you agree to indemnify and hold harmless LiveWell Medical and its owners, physicians, employees, contractors, representatives, and affiliates from claims, liabilities, damages, losses, and reasonable expenses resulting from:

  • Your unlawful or improper use of the Site;

  • Your violation of these Terms;

  • Your infringement of another person’s rights; or

  • Information or material you submit through the Site.

18. Privacy

Your use of the Site is also governed by the LiveWell Medical Privacy Policy.

Medical information maintained as part of an established patient relationship may also be governed by LiveWell Medical’s Notice of Privacy Practices and applicable healthcare privacy laws.

The website Privacy Policy is not a replacement for the Notice of Privacy Practices.

19. Children’s Use of the Site

The Site is intended primarily for adults seeking information about LiveWell Medical.

Individuals under 18 should use the Site only with the involvement of a parent or legal guardian. We do not knowingly invite children under 13 to submit personal information through the Site.

20. Governing Law

These Terms are governed by the laws of the State of Ohio, without regard to conflict-of-law principles, except where applicable federal law or another law that cannot be waived applies.
